PERMISSION TO PARTICIPATE IN WRESTLING

You will need to fill out and bring with you the following forms. All wrestlers must have one on file even if you are an adult. They are saved in PDF format which means you will have to have Adobe Acrobat Reader to view them. You can download it for free by clicking on the link below.

Prior to giving your consent parents should read this booklet about the sport of wrestling.
Parent's Guide to Wrestling

Get USA Wrestling Card as well as Freebirds Wrestling Uniform
Print this out and take it with you to practice. Give the money and form to Coach Hayhurst.
Check can be made out Freebirds Wrestling Club.

Emergency Consent Form Every wrestler must have this form on file with the coach at the local practice site.
