PERMISSION TO PARTICIPATE IN WRESTLING
You will need to fill out and bring
with you the following forms. All wrestlers must have one on file even if you
are an adult.

Prior to giving your consent
parents should read this booklet about the sport of wrestling.
Emergency Consent Form
Every wrestler must have this form on file with the coach at the local practice
site.
Open this form, fill out the information, then print it out and take it with you to practice. Give the money and form to
Coach Hayhurst or Coach Leach.
Check can be made out Freebirds Wrestling Club.
USA
Wrestling Membership Card
Every participant in the Freebirds club must have a USA
Wrestling Card of some type. This includes Coaches, Referees and Wrestlers.
Click on the above link to go directly to the registration site. Bring a copy of
the card with you to practice. It is very important that when you register you
select the following: For Association you must select "Southern California"
and for Club name type in "Freebirds Wrestling Club". If you start the
process you will get a number; write it down and don't forget it. This is your
membership ID#.
